COVID-19 Vaccination

Appointments are currently available to the following people with vaccination tickets

  • 12 year old and older people with vaccination tickets

Those under 16 years old will need to be accompanied by their parent / guardian to receive the vaccine. The parent / guardian will need to sign the medical questionnaire.

COVID-19 Booster Shot

When to get a booster shot

Yokosuka residents can receive their booster shot after at least 8 months have passed since receiving their second shot (some exceptions apply).

Who can get a booster shot

Residents registered in Yokosuka, 18 years old and older, who have received their second shots are eligible to receive their booster shot.

Vaccine types

The mRNA vaccine (Pfizer / Moderna) will be used regardless of which type of vaccine you have received for the first and second shot. Currently, the Pfizer vaccine is being adminstered as a booster shot.

Vaccination Certificate of COVID-19

Important information

  • Starting from December 20, the electronic version of the vaccination certificate is accessible through the smartphone app. In order to have your electronic certificate issued, you will need your My Number Card 4-digit PIN number. If you do not have a My Number Card, it may take about one month to have your My Number Card issued to you.
  • The hard copy of the vaccination certificate is also available. If you have trouble applying for the electronic version, please apply for the hard copy of the certificate.
  • If you already have a "Certificate of Vaccination for COVID-19" or the "Record of Vaccination for COVID-19," you can use it in place of this certificate in Japan.

Eligible people

Currently only available to people with residences registered in Yokosuka at the time of their vaccination.

The following people are ineligible to receive the Vaccination Certificate of COVID-19

  • People who have not yet been vaccinated
  • Those not covered under the Japanese Vaccination Law (e.g. those who have received a vaccine outside of Japan)
    ※ If you are a Japanese national or have residence status in Japan and received your vaccine from the U.S. military, please check the Ministry of Defense website (Japanese) for more information on how to obtain the vaccination certificate.

Necessary documents

  • Application Form for Vaccination Certificate of COVID-19
    Can also be obtained from the Yokosuka City Hall 1F vaccination reservation counter or from each administration center (Mon - Fri, 8:30 a.m. - 5 p.m.)
  • Copy of Certificate of Vaccination for COVID-19 or Record of Vaccination for COVID-19
  • Copy of proof of identification (My Number Card, driver's license, residence card, etc.)
    ※ Must contain information about your registered address
  • Required for certificates for "international travel & domestic use in Japan" only: Copy of a travel document such as a passport (copy of the page with the ID photo)
  • Return envelope with return address and postage attached
    ※ Requires 84 yen postage fee for standard mail. 120 yen for non-standard mail. Include required fees for any extra services such as express mail.

※ Write「新型コロナウイルス感染症予防接種証明書交付申請書 在中」in red on the envelope.

Notes

  • It may take 7-10 days for the certificate to be sent out.
  • If there are any errors on the application form, you may be contacted. Please include your telephone number.
  • The certificate is an A4-sized document.
