COVID-19 Omicron Vaccine Booster

Vaccination will only be carried out on a voluntary basis with consent. Those who do not wish to be vaccinated are not obligated to receive it.

Who can get an Omicron vaccine booster:

Any registered residents of Yokosuka who have completed a primary vaccination series (1st and 2nd vaccine) age 12 and older for whom 3 months or more have passed since their last vaccination are eligible to receive an Omicron booster.

*Residents who have received 3 or more COVID-19 vaccines are also eligible to receive an Omicron booster.

Please contact the Yokosuka City COVID-19 Vaccination Call Center (TEL 0120-432567) for available vaccination venues offering the Omicron vaccine booster.

Vaccination Schedule

You are eligible to receive an Omicron vaccine booster 3 months after the completion of your primary vaccination series or last booster. As different months have different numbers of days, you may need to roll over to the next month to count a full 3 months and become eligible.

*Vaccines must be those approved by Japan

 

Examples of how to calculate the 3 month waiting period

Omicron vaccine booster tickets will be sent to all residents age 12 and older who have completed their primary vaccination series. However, only the people eligible (as listed above) can receive the Omicron vaccine booster at this time. Non-eligible residents must keep their vaccination ticket safe for future use.

The vaccination ticket will arrive in a white envelope with a blue border at the bottom (as shown below). Please read the information inside and fill out the prevaccination screening questionnaire. You will need to bring the questionnaire and the vaccination ticket with you when you receive your Omicron vaccine booster.

 Vaccination of Children Age 5 to 11

COVID-19 vaccinations are now available to children age 5 to 11. They can receive three vaccinations of the Pfizer vaccine (a lower dose than the one used for age 12 and older). Each vaccine is given at least three weeks apart.

Is your child eligible for a third vaccination:

Children age 5 to 11 who are registered in Yokosuka, who have received their first two vaccinations and for whom 5 months or more have passed since their second vaccination are eligible to receive a third vaccination.

  • Yokosuka City Hall sent vaccination tickets for the third vaccination on September 16th to all children who already received their second vaccine.
  • Appointments must be scheduled directly with a participating medical institution. The Yokosuka City COVID-19 Vaccination Call Center can advise you on which institutions take reservations in English, but they cannot make the reservation for you. You can also consult with your family doctor.
  • Vaccination will happen on a voluntary basis, with the consent of both the child being vaccinated and the parents/guardians. Please consider the benefits and risks carefully before having your child vaccinated.

Things to do/bring on the day of your child's vaccination

  • Children will need to be accompanied by a parent/guardian during both the consultation and the vaccination.
  • Take your child's temperature before the vaccination. If your child has a fever or does not feel well, do not bring your child to their appointment. Instead, please contact the medical institution where you booked the appointment.
  • Wear a face mask and have your child wear clothing that makes it easy to expose and receive an injection in their upper arm.
Please bring the following documents with you:
  • Vaccination ticket
  • Prevaccination screening questionnaire (completed and signed by a parent/guardian)
  • I.D. for your child (e.g. My Number Card, health insurance card, special needs certificate)
  • Maternal and child health handbook (for checking your child’s immunization records)
  • Medication records (if applicable)

 Vaccination of Children 6 Months to 4 Years

COVID-19 vaccinations are now available to children age 6 months to 4 years. 

Is your child eligible

Children age 6 months to 4 years who are registered in Yokosuka are eligible to receive a Covid-19 vaccination course.

  • Yokosuka City Hall sent out vaccination tickets for children aged 6 months to 4 years on October 26.
  • The dose given to children is a special low dose, and they need a total of three shots to be considered fully vaccinated at this time. There must be at least 20 days between the first and second shot, and at least 55 days between the second and third shot. 
  • The vaccine that is given to children between 6 months to 4 years of age is different than the one given to children 5 to 11 years of age. Make an appointment based on the age your child will be the day of their vaccination appointment.

COVID-19 Vaccine (First and Second Shot)

Appointments are currently available to the following people with vaccination tickets

  • 12 years and older

※Residents aged 16 years old or older do not need the consent of a parent/guardian and to have a parent/guardian accompany them to their appointment. Residents age 12 to 15 years old will need the consent of a parent/guardian, as indicated by their signature on the prevaccination screening questionnaire, and may also need to be accompanied by a parent/guardian when receiving their vaccine, depending on the venue.

COVID-19 Vaccination Certificate

Important information

  • Starting from December 20, an electronic version of the vaccination certificate is accessible through the smartphone app. In order to have your electronic certificate issued, you will need your My Number Card 4-digit PIN number. If you do not have a My Number Card, it may take about one month to have your My Number Card issued to you.
  • A hard copy of the vaccination certificate is also available. If you have trouble applying for the electronic version, please apply for the hard copy of the certificate.
  • If you already have a "Certificate of Vaccination for COVID-19" (the one you receive when you get your injection in Japan) or "Record of Vaccination for COVID-19," these are generally accepted everywhere in Japan.

Who is eligible

Vaccination certificates are currently only available to those registered as residents of Yokosuka at the time of their vaccination.

The following people are ineligible to receive a COVID-19 Vaccination Certificate

  • People who have not yet been vaccinated
  • Those not covered under the Japanese Vaccination Law (e.g. those who received a vaccine outside of Japan)
    ※ If you are a Japanese national or have residence status in Japan and received your vaccine from the U.S. military, please check the Ministry of Defense website (Japanese only) for more information on how to obtain a vaccination certificate.

To obtain a hard copy of the COVID-19 Vaccination Certificate

Necessary documents

  • Application Form for COVID-19 Vaccination Certificate
    Can also be obtained from the Yokosuka City Hall 1F vaccination reservation counter or from a local administration center (Mon - Fri, 8:30 a.m. - 5 p.m.)
  • Proof you were vaccinated: Copy of "Certificate of Vaccination for COVID-19" (the one you receive when you get your injection in Japan) or "Record of Vaccination for COVID-19"
  • Copy of ID (My Number Card, driver's license, residence card, etc.)
    ※ Must contain information about your registered address
  • Required for certificates for "international travel and domestic use in Japan" only: Copy of a travel document, such as the ID photo page of a passport
  • Return envelope with return address and postage attached
    ※ Requires 84 yen postage fee for standard mail. 120 yen for non-standard mail. Include required fees for any extra services such as express mail.

Notes

  • It may take 7-10 days for the certificate to be sent out.
  • If there are any errors on the application form, you may be contacted. Please include your telephone number.
  • The certificate is an A4-sized document.
