COVID-19 2023 Autumn-Winter Boosters

Vaccination is recommended for those such as persons 65 years and older and those with underlying medical conditions who are at high risk of serious illness in the event of being infected by COVID-19. Therefore, please discuss vaccination with your doctor who is familiar with your personal health needs.

Furthermore, vaccination is not mandatory, so please make your own decision based on your understanding of both the benefits of vaccination and the risks of potential adverse reactions.

Who Can Get an Autumn-Winter Booster

All those who are age 6 months and older who wish to receive it can receive an Autumn-Winter Booster.

Vaccination Schedule

A vaccination campaign for those ages 12 and up is scheduled to run from Wednesday, September 20 until the end of March, 2024. Children ages 6 months to 4 years and 5 years to 11 years can be vaccinated starting October 2 until the end of March, 2024. (10/17/2023 Update: Campaign extended from the end of December to the end of March, 2024).

Type of Vaccine

This booster is an Omicron XBB vaccine.

Vouchers

The vaccination vouchers/tickets will arrive in the mail. Please read the information inside and fill out the prevaccination screening questionnaire. You will need to bring the questionnaire and the vaccination voucher with you when you receive your booster.

Precautions

• Take your temperature before going to your appointment. Do not go to your appointment if you have a fever or feel unwell. Instead, please contact the institution where you made your appointment.
• Wear clothing that will allow you to easily expose and receive an injection in your upper arm.
• The prevaccination screening questionnaire consists of two duplicate sheets. Fill out only the first sheet and bring the whole document with you, do not separate it in any way.
• Those who are age 15 years or under must have the consent of a guardian and a guardian must be present during the appointment. The signature of a guardian is also required on the prevaccination screening questionnaire.Some institutions may have restrictions about who qualifies as a guardian. Please inquire about any restrictions when making an appointment.

What to Bring to Your Appointment

Your vaccination voucher/ticket: Bring the entire document, do not separate it in any way.
Your prevaccination screening questionnaire: Fill out the required information before your appointment.
An ID: My Number card, driver’s license, health insurance card, etc.
Medication Records: If you are currently taking any medications, please bring your medication records. You may be asked to present them during your appointment.
Maternal and Child Health Handbook: If your child age 11 or under is going to be vaccinated, please bring your Maternal and Child Health Handbook (for checking your child’s immunization records).

 

 

COVID-19 Vaccination Certificate

Important information

Regarding the issuance of Vaccination certificates:

  • An electronic version of the vaccination certificate is accessible through the smartphone app. In order to have your electronic certificate issued, you will need your My Number Card 4-digit PIN number. If you do not have a My Number Card, it may take about one month to have your My Number Card issued to you.
  • A hard copy of the vaccination certificate is also available. If you have trouble applying for the electronic version, please apply for the hard copy of the certificate.
  • If you already have a "Certificate of Vaccination for COVID-19" (the one you receive when you get your injection in Japan) or "Record of Vaccination for COVID-19," these are generally accepted everywhere in Japan.

Who is eligible

Vaccination certificates are currently only available to those registered as residents of Yokosuka at the time of their vaccination.

The following people are ineligible to receive a COVID-19 Vaccination Certificate

  • People who have not yet been vaccinated
  • Those not covered under the Japanese Vaccination Law (e.g. those who received a vaccine outside of Japan)
    ※ If you are a Japanese national or have residence status in Japan and received your vaccine from the U.S. military, please check the Ministry of Defense website (Japanese only) for more information on how to obtain a vaccination certificate.

To obtain a hard copy of the COVID-19 Vaccination Certificate

Necessary documents

  • Application Form for COVID-19 Vaccination Certificate
    Can also be obtained from the Yokosuka City Hall 1F vaccination reservation counter or from a local administration center (Mon - Fri, 8:30 a.m. - 5 p.m.)
  • Proof you were vaccinated: Copy of "Certificate of Vaccination for COVID-19" (the one you receive when you get your injection in Japan) or "Record of Vaccination for COVID-19"
  • Copy of ID (My Number Card, driver's license, residence card, etc.)
    ※ Must contain information about your registered address
  • Required for certificates for "international travel and domestic use in Japan" only: Copy of a travel document, such as the ID photo page of a passport
  • Return envelope with return address and postage attached
    ※ Requires 84 yen postage fee for standard mail. 120 yen for non-standard mail. Include required fees for any extra services such as express mail.

Notes

  • It may take 7-10 days for the certificate to be sent out.
  • If there are any errors on the application form, you may be contacted. Please include your telephone number.
  • The certificate is an A4-sized document.
